IEEE Standards Association Fellowship Program

Welcome to the IEEE Standards Association (IEEE SA) Fellowship Program! IEEE SA is pleased that you have agreed to be a part of this program and looks forward to welcoming you.

To prepare for the logistics of your attendance, please fill out the IEEE SA Fellowship Registration Form and download a copy for uploading via our Submission Form along with the additional documents listed below.  Prior to doing so, please ensure that you have the following information ready:

• Downloaded and electronically completed versions of the following forms:
  • IEEE Vendor Add Form and Substitute Form W-8
  • IEEE Wire Transfer/ACH Payment Form
    • Please fill out the fields on the form from “Bank Name” to “Bank Account or IBAN#”
• Your brief biography (approximately 300 words), including your photo provided in Microsoft Word or an equivalent document format
  • In your brief biography, you should include
    • Information about yourself, including your technical expertise and your responsibilities
    • An overview of your standards experience (if applicable)
    • How attending the IEEE SA Fellowship Program will be relevant to your work
• Your preferred flight itineraries (please provide two choices per the IEEE SA Fellowship Program Agreement)
• A PDF/DOC/JPG of your visa (if you already have a valid visa for the meeting location)
